**Role Details**

The Manager, Care Service Pathway (CSP) is a role within the Exceptional Needs Unit (ENU) and is accountable to the General Manager, ENU for:

- Leading the implementation of the CSP across South Australia and providing oversight and leadership of complex and high need clients within the CSP.
- Leading advocacy and knowledge of the CSP cohort and working with stakeholders to achieve collaborative outcomes, consistent with best practice.
- Developing and maintaining high level productive working relationships with community stakeholders, clients and their families.
- Leading the delivery of highly effective support, including awareness raising and capability building, to community, government and non-government service providers including specialist referral and intervention services.

Key outcomes and accountabilities:
1. Contribute to strategic discussion at Federal and State level regarding current and future policy direction for the CSP cohort in relation to chronic health conditions and complex needs.
2. Build, manage and maintain positive and collaborative working relationships with peak advisory bodies, community leaders and identified stakeholders with awareness of political, cultural and security sensitivities.
3. Provide direction, empower, motivate and develop team members encouraging positive and consistent messaging in order to achieve Departmental goals.
4. Lead the CSP team in clinical and policy implementation of the program.
5. Maintain awareness of relevant trends, issues and best practice within the State, nationally and internationally.
6. Monitor and report to a wide range of stakeholders on the CSP.
Note: Any other responsibilities in line with the classification level of the role as assigned by Line Manager and/or the Department. The responsibilities as specified above may be altered in accordance with the changing requirements of the role.

Some interstate and intrastate travel may be required.
Must hold a current Australian issued driver’s licence (equivalent to minimum class C).
A tertiary qualification and/or demonstrated significant high-level experience in the field of social sciences, health management or other relevant discipline in a complex environment with competing demands is desirable.
