**Overview**

The **Women's & Children's Health Network** provides state of the art health care for women, babies, children and young people across South Australia. We are the state's leading provider of specialist care for children with acute and chronic conditions as well as being SA's largest maternity and obstetric service.

Our community-based services also reach hundreds of thousands of babies, children and young people, their families and communities across South Australia through our various services and programs in areas such as our Mental Health, Child & Family Health services, Youth Health and Aboriginal Health services.

We currently have an exciting opportunity to join the Aboriginal Continuity of Care Protocols (CCP) Program** **Team as **Aboriginal Senior Project Officer/Care Navigator** on **Full time (2FTE)**, **short term contract until 30 Jun 2024.**

The Aboriginal Senior Project Officer/Care Navigator is responsible for supporting the coordination and delivery of the activities agreed within the Continuity of Care Protocols Program agreement.

The Continuity of Care Protocols Program (CCP) aims to test continuity of care across the South Australian health systems through the Child and Maternal Continuity of Care Protocols Program, for children aged 0-4 years. The CCP Program addresses and reinforces the priorities of both Closing the Gap and Child & Maternal Health initiatives by focusing on how health system performance improvement can ensure that health care provided to Aboriginal people is well-organise, relationships are maintained, and information supports continuity of care regardless of how, where and when patients choose to receive care.
