About the Role

Our Care Consultants have experience with Case Management / Care Plans. You'll be visiting our clients in their homes located in Morwell, Sale and surrounding suburbs of Gippsland, to conduct assessments, care planning and ongoing client management.

Key Duties

Initial Customer assessments & service planning

Work with your customer to understand what they want from their Home Care Package, and assist them to build it based on their level of funding

Care planning, regular monitoring, and reassessment of customer needs

Regular visits to customers in their homes

Providing ongoing case management to HCP and or private customers

About You

You are passionate about continuing to grow your career in case management. Working as part of a close-knit team, you collaborate with and support others, demonstrating respect in your day-to-day actions.

This opportunity is working from home. You will be in the field 70-80% of the day, visiting customers.

**You will ideally bring the following**:
Qualification in Case Management, or

Undergrad qualification in Nursing / Allied Health / Social Work is essential

Div 1 Nursing Qualification - desirable but not essential

Prior case management experience will be highly regarded

Experience completing initial and annual customer assessments and care planning

Knowledge of the Home Care Package industry
