About the Role
We are seeking an experienced, enthusiastic community care professional to provide independent, ongoing advice and guidance to our NDIS clients across Melbourne. This is a full-time permanent role.
As an NDIS Support Coordinator, you will work collaboratively with Caring Hearts Home Care clients, their carers, and representatives, developing flexible, personalized, and culturally appropriate NDIS care plans. In this mobile team environment, you will:
- Review NDIS intake information, engage new clients, establish the NDIS service agreement, support plan, budget, and connect the client with appropriate NDIS-funded services and supports.
*Work mobile, conducting client visits as needed, practicing person-centered care planning, providing independent advice and guidance to NDIS participants and their families.
*Work collaboratively with participants, their carers, and representatives, ensuring their ongoing needs and preferences are supported to live independently and achieve their NDIS goals.
*Liaise with Caring Hearts Home Care employees, stakeholders, and NDIS service providers on behalf of the participant and Caring Hearts Home Care.
*Conduct annual NDIS plan reviews for all participants, revising their support plan, goals, services, and budget expenditure.
*Ensure all operations are conducted ethically, fairly, and in compliance with NDIS policies and contractual requirements.
*Attend our Head Office for NDIS training and team meetings.
About You
To be successful in the role, you will be able to demonstrate:
- Excellent interpersonal communication, networking, and negotiation skills within the NDIS framework.
*An ability to take initiative, prioritize, delegate, and organize activities while aligning with NDIS principles.
*In-depth knowledge of NDIS Care Plans, community resources, and service networks for participants and their carers.
*Minimum 2 years of experience with person-centered care planning processes in the NDIS, including assessment and establishing participant goals.
*Experience managing NDIS budgets, being responsive and accountable for time-sensitive tasks according to NDIS guidelines.
*Ability to work with set NDIS Key Performance Indicators (KPIs) and achieve targets.
*Proficiency in the Microsoft Office suite and willingness to acquire skills for NDIS-specific client database systems.
Qualifications required for the role:
*Tertiary qualifications in allied health, social science, disability studies, or a related field.
*In the absence of completed tertiary qualifications, experience in delivering person-centered practices in non-complex and/or complex NDIS support coordination or demonstrated experience with complex service-oriented offerings in disability services.
Additional requirements:
*A statutory declaration is required, confirming residency in a country other than Australia. Depending on the information provided, international police check(s) may also be required.
*Part of our selection process requires the completion of psychometric assessments.
