We have a **Divert Case Manager** role available in our **Toward Home Alliance (THA) **program. This role will be based at Terra Firma in the city.

Terra Firma is located in the Adelaide CBD and provides short-term accommodation for up to eleven singles or couples at risk of or experiencing homelessness, including two pet friendly rooms. The Terra Firma Case Manager works closely with residents, service providers and stakeholders to provide proactive case management that supports residents to secure safe, appropriate and sustainable long-term accommodation. This role requires a trauma-informed approach to assessment & intake, collaborative case planning, coordination of services, advocacy & support and community outreach with the aim of addressing the immediate needs of residents whilst working towards long-term solutions for housing stability and improved quality of life. This role is also responsible for site coordination of Terra Firma working closely with the THA Housing Allocations Coordinator and property managers, Harcourts Packham to support the management of the site.

**The duties of this role include;**
- Provide high quality, client-centred, strengths-based services using a trauma-informed and culturally responsive practice approach, adhering to the Divert Operations Manual as a best practice case management framework.
- Conduct comprehensive assessments of potential program participants, considering factors such as housing, health, psychological, cultural, and safety needs, including in-person assessments within the remand setting as needed.
- Develop and regularly update individualised case plans in collaboration with clients, addressing their specific needs and goals, and supporting transitions from short-term to stable accommodation.
- Implement and enforce facility rules and regulations to maintain a safe and respectful living environment for all residents.
- Conduct regular inspections of the premises to identify any maintenance or security concerns and promptly address them.
- Work within various Lutheran Care or Alliance Partnership sites located across the southern and CBD regions as required.
