Job Overview

The successful candidate will play a vital role in assessing, planning, and implementing services that address patients' health needs. You will primarily provide outreach nursing, focusing on wound care for patients in their own homes.

Key Responsibilities
  • Develop and implement comprehensive care plans for patients in their homes.
  • Consult with patients, families, caregivers, medical staff, and other healthcare team members regarding current medical needs.
  • Monitor and report any changes in patient status to relevant personnel to ensure timely and effective treatment.
  • Collaborate with all departments to uphold best practices for home patient care.
  • Work both autonomously and as part of a team.
Requirements
  • Current registration as a Registered Nurse with AHPRA.
  • A valid driver's licence.
  • A Current Working with Children Check.
  • Proven experience working within the acute and community nursing.
Salary and Benefits

The estimated annual salary for this position is $80,000 - $110,000 AUD, depending on experience. Cabrini Health offers attractive hourly rates, plus 11.5% Superannuation, salary packaging up to $15,900 tax-free each year, plus $2,650 meal & entertainment card benefits, and Myki Commuter discounted fares.
