**The fantastic opportunity**:
As the Clinical Assessment and Care Planning Coordinator, you will play a pivotal role in assessing the needs of our clients and creating tailored care plans to meet their unique requirements. You will collaborate with a multidisciplinary team of healthcare professionals to ensure the delivery of comprehensive and holistic care. Under the direction of the Homecare Manager, the A&CP Coordinator will support and manage the
Home Care Program assessments and care planning to ensure customers are enabled and supported to remain living independently in their own homes for as long as possible. The role is responsible for
ensuring that a high level of safe quality care and service is always delivered to customers.
The A&CP Coordinator will manage the assessment, support care planning, clinical governance, and delivery of services to customers within the home care legislation.
**About you**:
- Registered Nurse with current registration in Western Australia.
- Proven experience in clinical assessment and care planning, preferably in a home care or community setting.
- Strong interpersonal and communication skills, with the ability to build rapport with clients and collaborate effectively with a diverse team.
- A genuine passion for delivering client-centered care and improving the well-being of individuals in their own homes.
- Excellent organisational skills and attention to detail.
- Valid driver's license and access to a reliable vehicle.
