**_ The Role_**

The Coordinator Reablement (CR) will coordinate and deliver Reablement Services for clients assessed for this program.

Ensuring excellent communication is provided to other teams, including Customer Service Centre, Clinical Administrators (CA), Resource Coordinators (RC) and members of the multi-disciplinary team (MDT) including the clients GP, reporting directly to the Manager Reablement.

The Coordinator Reablement will undertake liaison with referral sources (ACAT teams) and endeavour to accept all referrals for the Reablement programs. Completing Initial Assessments, supporting the client to identify “Goals of Care” and developing care plans and therapeutic services to meet these goals. Conduct regular MDT meetings and case reviews as specified in the Short-Term Restorative Care Manual and the Transition Care Programme Guidelines.

Ensuring allocated funds are used appropriately to meet the objectives set in the care plan, maintaining the clients “budget planner” and making spending approval requests as identified.

The Coordinator Reablement works in partnership with internal stakeholders and other health professionals, clients, and their families to determine factors that effect, or potentially effect, the health and wellbeing of the client.

This position will be part of the Reablement Team, based at our Head Office in Subiaco working Full time; 76 hours a fortnight within Home Care.

**_ Key responsibilities:_**
- Responding to and triaging enquires received for the Reablement programs.
- Conducts Holistic Nursing Assessments that identify the client’s care needs and goals of care, providing effective personal and nursing care that is best practice, tailored to individual needs to optimise health and wellbeing, meeting the emotional, spiritual, and psychological wellbeing needs of clients.
- Works in partnership with MDT to provide reablement services ensuring appropriate care planning, reviews and regularity of scheduled services are implemented to meet client needs.
- Identifies and manages risk to provide effective service delivery.
- Treats clients and their families with dignity and respect while maintaining their privacy.
- Applies critical thinking and analysis to decision making following Nursing Standards for Practice
- Engages in ethically justifiable nursing practice.
- Advocates and protects the rights and privacy of older people.
- Completion of documentation and reporting as per the Short-Term Restorative Care and Transitional Care guidelines
- Completion of documentation in the Client Management System (Comcare) during and after the service
- Completing spending requests and approvals in Epicor
- Compliance with Relevant Acts and observed Regulations.
- Complies with Amana Living policies and procedures; and
- Manages regulatory compliance in occupational health and safety against Amana Living’s policies and procedures.
- Demonstrates regulatory compliance in Aged Care Quality Standards, Short Term Restorative Care and Transitional Care Guidelines and continuous improvement.
- Complies with infection control standards and practices.
- Functions in accordance with legislation and common law affecting practice.
