Why this role matters
The Salvation Army Australia’s Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

About the role
We are seeking a values-driven Homelessness Case Manager who will provide Case Management support to all cohorts experiencing or at risk of homelessness. Supporting individuals to achieve housing stability, improved social inclusion, connection to support systems and the wider community.
You will successfully
- Providing case management support to people experiencing, or at risk of, homelessness.
- Completing assessment for program suitability
- Case plans are developed alongside the participant and tailored according to each individuals' goals, background and needs; this is updated according to each contact/consultation.
- Evidence of flexible, proactive, client-centered approach including inclusive practice to case work with individuals' rights being upheld, interactions are respectful and free from discrimination, with cultural needs being catered for.
- Wrap around supports are implemented and lead by the client, monitored and reviewed. Follow up of supports are essential.
- Care plans are implemented and reviewed in a timely manner.
- Outreach meetings are carried out in relevant timeframes.
- Risk assessments and safety strategies are to be put in place prior to outreach support and home visits. Ensuring that the response is in accordance with the network protocols and complies with TSA risk management policies and procedures.
- Work within Trauma informed Care framework
- Client information, reports and data collection should be entered and recorded into the relevant database, consistently and in accordance with TSA, and SHS procedures and policies.
- Building Capacity and Education for corps, volunteers and community members in regards to Homelessness, pathways, prevention and early intervention
- Mandatory reporting processes are followed in a timely manner in line with legislation.
- Evidence of working effectively with internal/external stakeholders and related agencies.
- Attend internal and external stakeholder meetings.
- Stay up to date with all internal training.
- Participate and attend internal and external supervision.
- Contribute to a workplace environment which supports peers, develops teamwork and ensures the provision of quality services for TSA clients
This is a fixed term 12 Months role, based in Gosford. Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Award/General Retail Industry Award/Clerks Private Sector Award, Level 4.
You will have
- Tertiary or other qualifications within community services, social work or related discipline (Minimum Diploma Level, working toward a degree)
- Understanding of Trauma Informed Practice:
- Solid level of experience (minimum 2+ years) in the implementation of case management and systems for disadvantaged and/or complex needs clients.
- Experience of working with an inclusive practice, including with people from culturally and linguistically diverse backgrounds, LGBTQIA+, women, men & families is highly desirable.
- Experience working in collaboration with other organisations to develop and tailor programs to meet clients’ needs.
- Awareness of mandatory reporting guidelines is highly desirable.
