In this full-time position reporting directly to the Clinical Care Manager, you will be at the forefront of our home care operation, wielding your expertise and dedication to ensure its unparalleled success. **The Care Coordinator will be responsible for**:
- Building strong connections with clients and their families
- Assigning staff to clients based on staff qualifications and availability, as well as client preferences
- Organise support for clients and monitoring and reporting on their progress
- Referring clients to agencies that can provide additional help
- Assessing client needs and resources for health, welfare and other facilities and services
- Maintaining accurate records of incidents and feedback as required
- Re-organising work priorities to meet the needs of a fast-paced environment
- Supporting all clients to meet their goals
- Conducting Client Care Assessments and Care Planning
- Providing clinical guidance to other team members for client management
- Discussing and promoting Right at Homes services with medical professions and referral sources
- Ability to work in a fast-paced, dynamic environment
**Qualifications**:
- Qualified Nurse or Certificate IV
- Commitment to a culture of teamwork, compassion, and service excellence
- Strategic thinking abilities
- Understanding of the dynamics of a commercial operation
- Demonstrated leadership skills
- Strong judgment, quick decision-making, and effective staff management
- Excellent communication skills
- Some understanding of education and accreditation standards, Continuous Improvement, Occupational Health and Safety (OHS), and risk management
- Current National Police Clearance
- First Aid and CPR certificates
- AHPRA Registration (if applicable)
**Preferred Skills**:
- Communication skills (including patient compassionate listener)
- Computer skills (Office Suite, SharePoint, Teams)
- Coordination under pressure
- Interpretation and sound judgement
