About the Care Coordinator – Living Well Clinical Care Role

The Care Coordinator – Living Well Clinical Care position plays a vital role in delivering high-quality care and support services to older adults in the Capital Region. As a key member of the Living Well team, you will be responsible for assessing, planning, and coordinating services and supports to enable individuals to live safely and independently in their own homes.

Key Responsibilities
  • Supervise a team of care workers to ensure the delivery of high-quality services and supports to CHSP and HCP participants.
  • Manage participant plans and service delivery to ensure effective and efficient use of resources.
  • Conduct person-centered assessments, develop care plans, and coordinate services and supports to meet the unique needs of each individual.
  • Monitor and evaluate the quality of services and supports, identifying areas for improvement and implementing changes as needed.
  • Manage feedback and complaints, ensuring that all concerns are addressed in a timely and professional manner.
  • Ensure a safe and risk-free environment for participants, staff, and visitors.
Requirements
  • First Aid and CPR certification.
  • Certificate IV or Diploma qualification in Aged Care or a related field (or working towards or willing to obtain).
  • Working with Vulnerable People registration and National Criminal History Clearance.
  • Current Driver's Licence and access to a registered, comprehensively insured motor vehicle for work-related purposes.
