Then you could be the perfect Connections Transition Coordinator to join the professionals at Justice Health and Forensic Mental Health Network. Specifically, we’re looking for a healthcare professional that has obtained a degree in a relevant field, or equivalent work experience, or a combination of study and work experience. Someone with demonstrated experience in managing continuity of care for clients with chronic and or complex health needs. Here at Justice Health and Forensic Mental Health Network, the Connections Transition Coordinator (HM1) will be responsible for this project aimed to improve continuity of care for patients of the correctional centres with Drug and Alcohol problems, who are being released to the community. The Connections Program works within an assertive linkage model that aims to link patients with relevant health and welfare service providers post release. The Connections Program team utilises a broad array of contacts, both in the correctional and community environments to assertively link patients into services appropriate to their individual needs. The role and responsibilities are to be carried out in a manner that is consistent with Network delegations, policies, procedures and operations systems, and in line with both the NSW Health and Network Codes of Conduct.

- A degree in a relevant field, or equivalent work experience, or a combination of study and work experience.
- Demonstrated experience in managing continuity of care for clients with chronic and or complex health needs.
- Demonstrated competent written and oral communication skills including computer literacy.
- Demonstrated experience of working with community based networks and demonstrated understanding of Drug and Alcohol treatment approaches.
- Demonstrated ability to work both independently and as part of a multi-disciplinary team and meet tight deadlines.
- Demonstrated good level of conflict resolution and negotiation skills and proven networking and excellent interpersonal skills.
- Must have a current drivers license and willingness to travel
