**The Role***
The Connections Program team aims to improve continuity of care for patients of the correctional centres with Drug and Alcohol problems, who are being released to the community. The Connections Program works within an assertive linkage model that aims to link patients with relevant health and welfare service providers post release.
The Connections Transitions Coordinator is responsible for following up on referrals received and ensuring that:
- Prior to a patients release, a comprehensive assessment and post release care plan are in place to addresses the patients individual needs in line with the Connections Program Operations Manual.
- The patient's current health records are reviewed and all identified and active health concerns are addressed.
- Liaison is made with relevant health and welfare service providers, to ensure the smooth transition of care.
- Patients are assertively followed up in the community during their time on the Connections Program post release in line with the Connections Operations Manual.
- Represent JHFMHN at a range of meetings both internal and external to the organisation that are relevant to the position.
- Maintain regular contact with the Manager Connections Program and report any issues of concern that may arise.
- Other duties as directed by the Manager Connections Program or the Manager D&A Release Planning

- Demonstrated experience in managing continuity of care for clients with chronic and or complex health needs.
- Demonstrated competent written and oral communication skills including computer literacy.
- Demonstrated experience of working with community based networks and demonstrated understanding of Drug and Alcohol treatment approaches.
- Demonstrated ability to work both independently and as part of a multi-disciplinary team and meet tight deadlines.
- Demonstrated good level of conflict resolution and negotiation skills and proven networking and excellent interpersonal skills.
- Must have a current drivers license and willingness to travel.
