**About the opportunity**:
**_The Programs_**:

- HACC-PYP has been designed to support people with disabilities, a medical or health condition and their respective carers to live as independently as possible in their own homes. The program supports people aged under 65 and Aboriginal people aged under 50 who need assistance with daily activities, including personal care, dressing, preparing meals, house cleaning, property maintenance, community access and using public transport.
- The Support for Carers Program provides person centred respite and suitable support and services that meet the needs of Young carers outside of the National Disability Insurance Scheme (NDIS).

**_ The role_**:

- The Intake & Assessment Facilitation Officer will complete holistic assessments of need to support the development of goal directed support plans promoting a strengths-based approach in partnership with service providers to ensure that individual client needs can be met.

**About you**:
To be successful in this role, you will have:

- A tertiary qualification in Social Sciences, Disability Studies or Allied Health professions, or equivalent qualifications with demonstrated industry experience.
- Current First Aid and CPR certificate
- Understanding of the issues related specifically to the caring role
- Experience or knowledge regarding the provision of care to the elderly, younger people and people with disabilities
- Understanding of privacy and confidentiality obligations
- Knowledge of principles of community inclusion and participation, with experience in delivering a response or project as a result to a community need or service gap
- Demonstrated experience in performing community and/or health assessments using a strengths-based approach, and development of associated care plans
- Experience working collaboratively with clients, carers and/or relevant agencies to achieve client goals.
