**About the Role...**

We have an exciting opportunity for two passionate and community focused individuals to join our Positive Ageing team. We are recruiting for 2 positions, 1 Full Time, and 1 0.6FTE both contracted to 30 June 2024.

The CHSP Intake and Review Officer will support with monitoring and actioning client referrals, undertaking the client intake assessment and planning through the My Aged Care portal and Council’s Customer Management System to meet service, organisational and team objectives.

This position is responsible for undertaking the client service review process including identifying and actioning appropriate service referrals to support client-centric outcomes including wellness and reablement goals.

Incumbents in this position may be required to possess or be willing to gain a National Criminal History Clearance.

You will have excellent communication skills, both verbal and written, with the ability to provide quality customer service to a diverse range of people.

In addition, you will have experience in aged care service intake and review, excellent time management skills and the ability to plan, prioritise work and meet deadlines.

Qualifications or experience in Community Services and an understanding of the Commonwealth Home Support Programme (CHSP) is desirable.
