About the role
We are seeking a Social Worker to join our Social Work team in General Medicine at The Box Hill Hospital.
You will have relevant hospital experience and be able to work within a strong multidisciplinary team.
It will be essential to have a sound understanding of complex discharge planning within an acute hospital setting in particular an understanding of Transition Care Program, My Aged Care and the NDIS.
About you
**To be successful in this role you will possess**:
Holds a degree qualification in Social Work that has been approved by the Australian Association of Social Workers (AASW) for membership as a practicing Social Worker.
Maintains compliance with the AASW Continuing Professional Development Policy in order to meet the Eastern Health Allied Health credentialing standard.
Demonstrated capacity to work effectively in a fast-paced health environment
Excellent problem-solving skills
Demonstrated capacity to work effectively in a multi-disciplinary team
Excellent communication skills
Basic knowledge of the relevant legislation and frameworks
