Your Side Australia is a leading provider of support services for older people, people living with disability, mental health, and carers throughout Sydney. Established 30 years ago, we've kept our commitment to giving people greater choices and enhancing their independence and well-being. Your Side is a one-stop shop to arrange the care and support that allows people and families to get on with living a good life.

The care finder program delivers a national, face-to-face network of care finders. It provides support for vulnerable older people to interact with My Aged Care, access aged care services, and access other relevant supports in the community. The program aims to target harder-to-access groups in the community who are unlikely to engage with the system due to barriers such as language, cognitive decline, or a reluctance to deal with the government systems.

**_The Role:_**

Work for a reputable NFP in the Care Finder Program to perform a blend of intake and assessment.
- Take queries from the above-mentioned targeted groups who are struggling to engage with the aged care system.
- Build trust in vulnerable clients using trauma-informed and client-centred practice.
- Promptly answer basic queries and record actions on files.
- Refer ineligible clients to other support services.
- Pass eligible clients to a relevant Care Finder within 24 business hours.
- Support Team Leader by collating information for monthly, weekly, and quarterly reporting, including referrals received and made, and caseload sizes.
- Assist with audit preparation.
- Keep records that mean that program reporting requirements are met including funder and Your Side reporting in our CRM and any other required sources.
- Assist with recording and analysing client feedback.

**_About yourself:_**
- You have experience with intake and assessment.
- You have a person-centered approach and are trauma-informed.
- You have experience using Excel, Word, and CRMs.
- You have a background or interest in working with one or more of the key target communities or groups; CALD, LGBTQIA+, Aboriginal and Torres Strait Islanders, homelessness, victims of DV, and AOD.

**_Desirable but not required:_**
- You have or are in the process of acquiring qualifications in community services, social work, or another relevant field.
- You have knowledge of the aged care space.
- Speaking multiple languages would be highly regarded

**_Requirements for the role:_**
- Double vaccinated against COVID-19 OR valid medical exemption
- Able to conduct Police Check
