Why this role matters
The Salvation Army Australia’s Homelessness Stream provides high-quality, person-centred services to people at risk, and or experiencing homelessness.
Foster House offers a safe, welcoming, and inclusive environment to some of the most vulnerable and disadvantaged people experiencing homelessness in the inner city of Sydney. The 24/7 hour service supports fifty single men in a residential setting. The team works in line with Trauma Informed Care principles, the No Wrong Door Policy, and The Salvation Army’s Mission, Vision & Values at all times.
About the role
We are seeking a values-driven and proactive Case Manager to provide case management support to men and families experiencing homelessness, helping them achieve housing stability, improve social inclusion, and become equipped for participation in the wider community.
Reporting to the Team Leader, this is a permanent, full-time position based in Surry Hills. Salary and conditions are in accordance with the SCHADS Crisis Award Level 2.
You will successfully- Support the implementation of the Homelessness Stream Model of Care that underpins services across the country and defines expectations and standards for those we serve, our partners and allies, and the Australian community.- Develop Case plans in consultation with the people seeking support, and tailor them according to each person’s needs and background.- Follow mandatory reporting processes in a timely manner in line with legislation.- Work positively and effectively with others in the multi-disciplinary team as well as external community services and related agencies.- Undertake appropriate risk assessments and safety planning prior to conducting all outreach and offsite activities.- Organise and coordinate onsite activities and events with residents and the people we support.- Contribute to a workplace environment which supports peers, develops teamwork and ensures the provision of quality services for TSA clients.
You will have- A NSW Working With Children Check.- A valid NSW Driver Licence (preferred).- Tertiary or other qualifications within community services, social work, or related discipline (Minimum Diploma Level, working toward a degree).- An understanding of Trauma Informed Care Frameworks and Practice (desirable).- Sound experience (minimum 2+ years) working in the community service sector (desirable).- Flexibility with work hours to meet the operational requirements of the position.- Experience in implementing case management and systems for disadvantaged and/or complex needs clients (desirable).- Experience or a demonstrated understanding of people experiencing homelessness, experiencing mental health and people with AOD issues is desirable.- Experience working with people from culturally and linguistically diverse backgrounds and religions is desirable.- Experience working in collaboration with other organisations to develop and tailor programs to meet clients’ needs.
