**Overview**
- The Education Facilitator, Transition to Practice Coordinator is responsible for the coordination, planning, development and evaluation of professional development activities for nurses involved in all transition to practice programs across the Women's & Children's Health Network (WCHN).
- The Education Facilitator, Transition to Practice Coordinator must demonstrate the necessary leadership, management, education and clinical skills fundamental to a learning organisation in a multidisciplinary environment, whilst also demonstrating effective and efficient use of all resources.
- The Education Facilitator, Transition to Practice Coordinator will ensure that services are continually evaluated and improved, and participation of consumers is maximised.

**About the Women's and Children's Health Network**

The Women's and Children's Health Network provides state of the art health care for women and children in Adelaide and South Australia.

It is the state's leading provider of specialist care for children with acute and chronic conditions and SA's largest maternity and obstetric service. It also provides important community-based services.

Each year, more than 30,000 people are admitted and about 5000 babies are born at the Women's and Children's Hospital. In addition more than 250,000 people come to the hospital as outpatients.

The hospital has 295 beds. The hospital's paediatric and adolescent wards cater for all paediatric specialties. Women's wards provide antenatal, gynaecology, neonatal and postnatal care.
