You will be working with NDIS participants to build capacity and implement support to help them achieve their goals outlined in their plans.

The role of a support coordinator is to connect participants with an NDIS plan to supports and services in the community. Your role will require that support coordination is completed as agreed, within a timely work schedule and that claims for payment are lodged. This role assumes full responsibility for ensuring that each participant’s plan is implemented, and the participant achieves their goals outlined.

You are expected to present a positive and professional image of DMS when interacting with participants, family members, advocates, service providers, funding bodies and the general community.

**DUTIES AND RESPONSIBILITIES**:
Support Coordination
- Build NDIS participant and family capacity to understand and navigate service system
- Support participants and carers to understand their NDIS Plan, goals and objectives
- Ensure all organisational and administrative reporting and accountabilities are completed accurately and in a timely manner
- Contact the client as soon as possible after the handover with the NDIS planner, ideally within 2 days and meet with the client within 5 days
- Deliver coordination of supports to individuals with an NDIS plan:

- Research, coordinate and manage complex and intersecting supports to suit individuals needs across multiple providers, ensuring client choice & control
- Support participants to build their capacity, resilience and networks with the aim of greater choice and control and independence in managing their personal choices
- Work with participants to access and navigate the My place portal to establish service bookings and service agreements
- Arrange any assessments needed to determine the nature and type of funding required
- Decide the budget for each support type and advise any relevant plan manager of breakdown of funds
- Liaise with plan managers to establish the appropriate claim categories and attribute funds accordingly
- Assist participants to prepare for plan reviews by helping them asses whether they achieved their goals and got value for money for their plan, identify solutions to problems experienced in implementing the plan, and to consider new goals
- Assist participants to communicate with the NDIA when required to support their own plan requirements
- Resolve any crisis situations, ensuring that the participant best interest and resilience are supported
- Ensure the completion of NDIS participant service agreement and other appropriate documentations withing the initial assessments as required
- Ensure PRODA service bookings are completed for each participant
- Maintain documentation to demonstrate the service procedures are followed
- Monitor and report participant outcomes to the NDIA through progress reports
- Actively participate in the development, implementation, monitoring, regular review and reporting of participant programs with providers including determining risk and preparing assessments, review participant records and understand individual needs, review participant goals and measure individual progress and document outcomes
- Develop and execute strategies for individual participant retention

**NDIS SERVICE PROVIDERS**
- Work with mainstream service providers including negotiating services and prices to ensure maximum value for money is achieved and service obligations are met
- Assist with recruitment of service providers
- Assist with the continual evaluation of service provider range of services and quality
- Obtain, retain and check validity of service provider qualifications/acreditation
- Maintain register of approved and preferred service providers
- Adhoc problem solving with Service Providers in relation to participant needs
- Provide feedback to Operations Manager regarding Service Providers and Participants

**NDIS, NDIA, WORKPLACE HEALTH AND SAFETY AND QUALITY**
- Execute Support Coordination within NDIS and NDIA policies, procedures and requirement including completion and submission of paperwork to ensure Support Coordination and DMS compliance
- Assist the Operations Manager as the key contact for external Support Coordination audits
- Advise risks and recommendations from risk assessments to Operations Manager
- Maintain up to date knowledge of NDIS and NDIA requirements
- Adhere to DMS policy and procedures, including Workplace Health and Safety
- Adherence to the developing quality sytems of DMS
- Undertake training in policies, procedures and NDIA requirements to support applicants and participants as advised by Operations Manager
