Beaumont People are thrilled to be partnering with a dynamic community services organisation in search of a leader in the Aged Care sector, who is passionate about engaging with the ageing community to build stakeholder relationships and coordinate creative social support activities which promote social connectedness and belonging.

**Kogarah Community Services**

Kogarah Community Services (KCS) is a diverse, independent not-for-profit organization dedicated to providing services for disadvantaged and vulnerable people within the South-Eastern Sydney area. With a fundamental purpose to make a positive, lasting difference in the lives of those in need, KCS work collaboratively with a range of community stakeholders to develop and deliver exciting and inclusive programs to strengthen communities and support vulnerable people. These programs are tailored for children and families, elderly citizens, and minority groups.

As an Australian Government approved Aged Care service provider, KCS have also been delivering outstanding Aged Care services for seniors for over 25 years. The activities they deliver aim to encourage and support older individuals to remain active, engaged, and connected with their community. These services include the Commonwealth Home Support Program (CHSP), Home Care Packages (HCP), and Private Services.

**In this role, you will**
- Manage a case load of existing and new CHSP clients, involving intake and assessment, home visits, and connecting clients to Social Support services and activities.
- Identify the unique social support needs of CHSP consumers and develop creative activities which address these needs and promote a wellness and reablement approach.
- Build and maintain relevant stakeholder relationships and conduct networking activities which promote and enhance the Social Support program offered by KCS and promote the organisation in the community.
- Collaborate with the Community Engagement team to develop innovative and creative programs and social support activities for CHSP consumers which promote social connectedness and belonging.
- Support and mentor staff and volunteers, ensuring a high-quality provision of service.
- Be proactive and take initiative to identify new ways of growing the program.
- Actively promote and represent the program and the organisation to the local community.

**To have success in this role**, you will be an enthusiastic, practical and creative leader with strong community services experience and a passion for engaging with a variety of community stakeholders.

**You will also have**
- Relevant tertiary qualifications AND/OR substantial experience in Aged Care, Disability, or community services (experience working with CHSP clients is highly desirable)
- Leadership or management experience or demonstrated ability to effectively supervise and mentor a small team of staff and volunteers.
- Strong case management experience and ability to manage a caseload and deliver effective client services to the ageing community.
- Experience developing, coordinating, and facilitating community programs, initiatives and activities which are engaging and creative.
- Excellent communication and networking skills, and ability to build strong relationships and engage with clients, staff, and external stakeholders.
- A proactive and innovative approach to program development and promotion

- A valid driver’s license and vehicle with comprehensive car insurance
- A recent Police check or willingness to obtain.
- A valid Working With Children’s check or willingness to obtain.
- A valid NDIS Workers Screening Check or willingness to obtain.
- A Flu Vaccination
