Aligned with CALHN's strategic vision for delivering world-class healthcare, Hampstead Centre is launching a new program, “CO-ACT” (Care of the Older Person and Community Transition), focused on providing exceptional care for older adults transitioning from acute to ongoing care. As a member of our CO-ACT team, you'll work in a supportive, family-like environment where collaboration is key, and your input on improving systems and practices is highly valued.
