**About the role**

An exciting opportunity exists for a motivated individual to join the highly successful Hospital Elder Life (HELP) team at Grampians Health - Ballarat (GH-B). HELP is a world-renowned delirium prevention, management and treatment approach which was originally developed at Yale University in 1999. Since then, HELP has expanded to 17 countries and more than 200 sites worldwide, and led to the development of Geriatric Centres of Excellence. HELP at GH-B commenced at the acute service in mid 2021 and has expanded to incorporate five wards at the site. The Elder Life Assistant with support the Program Manager to screen and enrol patients into the program; orient patients and families to the program; assign appropriate intervention protocols; develops and oversees implementation of appropriate volunteer assignments for patients; provides administrative support to assist with the recruitment, screening and placement of program volunteers.

**Key Responsibilities**
- Daily screening and enrolment of appropriate patients into program.
- Orient patients and families to the Program.
- Prepare and update individual care plans for the HELP intervention.
- Assist in the oversight of the patient care assignments.

**Skills and experience**
- Qualification in Community Development, or Health-related area would be highly regarded.
- Knowledge of Hospital Elder Life Program (HELP) is desirable and previous experience working with HELP is preferred.
- Excellent interpersonal skills and flexibility.
- An interest in working with a diverse group of people.
- A commitment to high-quality person centred care, with previous interest in geriatrics, delirium and/or volunteering being highly desirable.
- Strengths in multi-tasking and attention to detail.
