- Nepean Hospital is a teaching hospital that provides tertiary referral services in emergency care, coronary care, diagnostics, gynaecology, paediatric, neonatal intensive care/ICU, maternity, mental health, rehabilitation, and surgery. The Nepean Campus has recently experienced tremendous growth and aims to support communities within and outside of Sydney’s western suburbs, providing world-class clinical facilities, services, and care.

Our Aged Care Services Emergency Team is growing and is seeking to engage a qualified Social Worker in a Temporary Full Time assignment. The Social Worker will provide a high quality social work service to the Aged Care wards at Nepean Hospital. Key to the success of this role requires working knowledge of social work theories which inform your practice.
The successful incumbent will also be required to work closely with the multiple disciplinary teams (MDT) on the ward/s to achieve best outcomes for our patients in an efficient and effective way. Members of our Age Care Social Work team are required to complete psychosocial assessments, attend case conferences, facilitate family meetings, provide varying types of counselling and contribute to case and discharge planning.

**What will you bring to the role**:

- Tertiary Qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW)
- Demonstrated understanding of Aged Care Social Work within a Hospital environment
- Demonstrated high level communication, report writing and computer skills
- Demonstrated ability to work independently and as a part of a Multidisciplinary team.
- Demonstrated understanding of psychosocial stressors that could impact on the admission.
- Basic knowledge of My Aged Care, Carers Gateway, NSW Civil Administrative Tribunal - Guardianship Division, National Disability Insurance Scheme, the Ageing and Disability Commission and Residential Aged Care.
- Demonstrated capacity to Link theory and practice
- Current unrestricted driver's license and willingness to travel for work.
