**Overview**

A great opportunity is available for a Mental Health Hospital Liaison Clinician with a passion for making a positive impact in the lives of people with mental health challenges, to join our dedicated Mental Health team at Wallaroo Hospital.

In this role, you will provide:

- specialist Mental Health assessment,
- intervention and discharge planning for Mental Health Patients in acute settings,
- Mental Health training and education,
- Mental Health Quality Improvement,
- alternate care pathways,
- improve patient outcomes, and
- reduce the number of Wallaroo Hospital Emergency Department (ED) presentations.

**Experience required**:

- Previous experience in hospital liaison role
- Ability to work across both Emergency department and acute inpatient setting to support patient journey
- Ability to safely implement hospital avoidance strategies where appropriate
- Ability to work within multi-disciplinary team taking into consideration site needs and capacity
