- Develop and review comprehensive goal-directed care plans and risk assessments in consultation with clients, their nominated representatives, and relevant stakeholders within individual client budget parameters.
- Regularly monitor the progress of clients and evaluate the effectiveness of interventions, making necessary adjustments to the service plans as needed.
- Investigate and respond in a timely manner to consumer enquiries, concerns or complaints and ensure that consumers are advised of the agreed level of services and any changes to plans.
- Advocate on behalf of clients to ensure they receive the necessary services, accommodations, and support to enhance their independence and well-being.
- Ensure that clients' rights and preferences are respected, and advocate for their inclusion and integration in the community.
- Educate clients and their families about disability-related issues and available services while providing emotional support and guidance.
- Assist clients in managing their budget, including accessing disability benefits and social services.
- Collaborate with other professionals, such as healthcare providers, social workers, educators, and community agencies, to ensure a holistic and coordinated approach to care.
- Team Management: Assign tasks and responsibilities to team members as well as define team goals and objectives and set clear performance objectives and targets.
- Review shift reports and incident reports and provide guidance or support to other frontline workers when required.
- Stay up to date with relevant laws, regulations, and best practices in the field of disability services.
**Essential Requirements**
- A current and valid driver’s license.
- Proof of documentation for valid working rights in Australia.
- Knowledge of NDIS guidelines and policies.
- Proven experience working with individuals with disabilities.
- One to three years working as a Case Manager.
- A strong ability to advocate for the rights and needs for clients.
- Experience in managing staff and work placement students.
- Excellent communication and interpersonal skills.
- Empathy and patience when working with clients with diverse needs.
- Organizational skills for managing caseload and documentation.
**Desirable**
- Experience working in a hospital environment will be highly regarded.
- Knowledge and experience working with people with complex mental health conditions will be highly regarded.
**Qualifications/Certification**
- A Bachelor’s or Masters Degree in social work, psychology, or a related field is essential.
- NDIS Clearance check or the ability to obtain one.
- NSW Working with Children Check or the ability to obtain one.
**Base pay**
$65000 - $75000 + Super
