
CCCP Care Coordinator

In this highly rewarding role, you will provide care coordination, intensive case management and specialist support to clients seeking assistance with AOD use and other complex needs. You will complete person-centred assessments and work alongside clients to develop tailored care plans that support them in achieving their goals. Based in Wyong, this dynamic role involves providing support through a mix of outreach, office-based and virtual services. You'll join a collaborative and inclusive team, working to create positive outcomes for individuals with complex support needs.
Your Responsibility- Collaborate with referring staff and external agencies
- Develop and maintain referral pathways to health and non-health community services
- Provide culturally appropriate care for Indigenous clients, including strong service linkages
- Maintain accurate case notes, records and data
- Provide support across employment, education, daily living, housing, social connection and community or family reconnection
- Relevant tertiary qualification or VET qualification with substantial experience
- Experience supporting clients with complex needs such as AOD use, mental illness, trauma, homelessness or justice system involvement
- Skilled in assessment using standard tools and delivering therapeutic interventions
- Professional, ethical and non-judgemental in approach
- Able to work independently and collaboratively
- Sound knowledge of local services and resources
- Confident with Microsoft Office and comfortable managing case data
- Current NSW Driver Licence
