Senior Child Health Liaison Nurse

This is a unique opportunity to support the health of kids in care by coordinating assessments and plans in a liaison role across agencies in Western Sydney.
The OOHC Health Pathways Program is a key initiative within Western Sydney Local Health District (WSLHD).
Main Responsibilities:- Facilitate and implement health assessments and health management plans for children and young people entering Out of Home Care (OOHC) within WSLHD.
- Work within a multidisciplinary and multi-agency approach with internal and external stakeholders, including government and non-government partners.
- Clinical, liaison, implementation and monitoring functions to support OOHC health needs.
- Current Registered Nurse Division 1 with APHRA
- Minimum of three (3) years post-registration experience appropriate to performing the role of a Clinical Nurse Specialist Level 2 for children and young people in OOHC
- Currency of practice and competence in assessing children and their families in relation to health and social needs; understanding of Families NSW principles
- High level communication, liaison, networking and negotiation skills
- Understanding and ability to implement primary health care principles and work in partnership with families
- Experience working collaboratively in a multidisciplinary team with other service providers
- Ability to work independently with strong time management and problem-solving skills
