What you'll do

Deliver warm, phone-based care support to a dedicated portfolio of older Australians

Partner with clients to create, review and adjust personalised care plans

Empower clients to self-manage their Home Care Packages with confidence

Build trusted relationships with clients, families and service providers

Support safe, high-quality care through proactive risk awareness and documentation

Help clients navigate their care budgets with clarity and transparency

Contribute to a positive team culture through shared learning and innovation

What you'll bring

Experience in aged care, NDIS, case management or community support

A proactive, organised approach with the ability to manage multiple tasks

Strong communication skills, including confidence in handling sensitive or complex conversations

Confidence using technology and working across multiple systems

Highly proficient in Microsoft 365 and confident navigating CRM and documentation systems

A calm, constructive mindset when things don't go to plan - skilled in problem-solving and helping others feel heard

A belief in empowering people rather than doing everything for them

A team-first attitude with energy, empathy, and initiative

(Experience in aged care or case management is a bonus, but not essential. We'll teach you what you need to know)
