About the role

Mercy Community is seeking an experienced Clinical Care Partner to join our Community Aged Care Services team in Rockhampton on a Permanent Full-time basis.

The Clinical Care Partner holds clinical oversight under Community Care, responsible for delivering clinical care management and oversight to enable older people to live safely and independently at home. Working within a rights-based, person-centred framework, the Clinical Care Partner collaborates with participants, their supporters, and interdisciplinary teams to coordinate tailored, responsive, and inclusive support.

  • Salary commensurate with experience plus superannuation and salary packaging options

This role involves, but is not limited to:

  • Collaborating with participants to develop individualised care plans within funding timeframes
  • Incorporating the participant's goals, service preferences, cultural needs, risk factors, and environmental requirements into care plans and delivery
  • Establishing and updating care plans and funding requirements, ensuring participants understand their available services
  • Working as part of a team-based model, providing clinical care management and guidance to multidisciplinary teams

About you

As the successful applicant, you will possess:

  • Bachelor's degree in nursing, social work, allied health, or similar
  • Demonstrated experience in care coordination, case management, or similar roles in aged care, health, or disability services
  • Solid understanding of the Support at Home Program, Quality Standards, and aged care rights
  • Competence in interpreting clinical guidance and escalating appropriately
