About the Role: Transient & Outreach Manager

The Transient and Outreach Manager will be responsible for leading a team in the development, implementation, and evaluation of a comprehensive primary health care service aimed at assessing and stabilising the immediate health care needs of vulnerable homeless and transient individuals in the Greater Darwin region.

This initiative will involve a multidisciplinary team approach to address both health and social support requirements with a focus on facilitating continuity of care for individuals within the Darwin region and supporting their departure from the region.

What You Need to Succeed

Minimum 3 years' experience in project management, preferably in the healthcare or social services sector.

  • A formal qualification in a related field such as healthcare administration, healthcare management, nursing, public health, or a clinical discipline.
  • Previous experience working in the Aboriginal Health Sector.
  • Understanding of current health issues relevant to Aboriginal and Torres Strait Islander communities, particularly with chronic disease.
  • Demonstrated knowledge of primary health care principles, multidisciplinary team approaches, and social determinants of health.
  • Strong management, leadership, and interpersonal skills, with the ability to effectively collaborate with diverse stakeholders and build partnerships.
  • Excellent communication skills, including written and verbal communication, presentation, and negotiation skills.
  • Experience in program evaluation, data analysis, and performance measurement.
  • Ability to work independently, prioritise tasks, and manage competing priorities in a dynamic environment.
  • Understanding of cultural competency and sensitivity to the needs of diverse populations, including Indigenous communities.
  • Knowledge of relevant legislation, policies, and guidelines related to healthcare service delivery in Australia.
