Aboriginal Health Practitioner – Transient And Outreach

Tasked with conducting thorough health assessments and addressing immediate needs, the AHP collaborates closely with a multidisciplinary team to ensure a comprehensive approach to health and social support.
Emphasising continuity of care within the Darwin region, the AHP facilitates smooth transitions for individuals departing the area, or accessing other services, advocating for their health and social welfare.
What You Need to Succeed Certificate IV Aboriginal and Torres Strait Islander Primary Health Care Practice and current registration with AHPRA.
Minimum 3 years' experience working in a Primary Health Care setting.
Knowledge of the social determinants of health (environment, economic and social inequalities) that have a profound impact on the health and wellbeing of Aboriginal and Torres Strait Islander people.
Well-developed knowledge of, and contemporary experience in, working with clients with chronic health conditions (e.g., diabetes, cancer, respiratory and / or cardiac disease) and relevant management strategies.
High level of personal resilience and flexibility with the ability to deal with challenging situations.
Demonstrated competence working within multi-disciplinary teams or independently, with strong organisational skills and the ability to plan, manage and prioritise tasks.
