About the Role:

We are excited to be expanding our Urgent Community Response (UCR) service within the Community Frailty Hub in York. Were looking for two Intermediate Clinical Practitioners to join our dynamic and supportive multidisciplinary team.

This is a fantastic opportunity for motivated and compassionate clinicians looking to develop their skills in urgent care, frailty management, and community-based care. You will play a key role in providing rapid response to patients with frailty-related health concerns, helping to avoid unnecessary hospital admissions by delivering timely, person-centred care in the patients usual place of residence.

Main duties of the job

Key Responsibilities:

  • Deliver urgent two-hour response assessments and interventions for people living with frailty in their own homes.
  • Support care delivery under the supervision of advanced clinical practitioners or GPs.
  • Work as part of a wider multidisciplinary team (MDT) including GPs, nurses, therapists, and social care professionals.
  • Maintain a clinical portfolio and contribute to service improvement.
  • Work flexibly across a 7-day service (8am8pm) in line with rota requirements, including some weekends.

Nimbuscare deliver Urgent Care services in collaboration with the hospital and GP practices in York, Scarborough, Whitby, Malton and Selby. We also provide community-based frailty care, which focuses on treating people in the community where we can. We also aim to shift services into the community where possible, for example our community diagnostic centres run in partnership with the Acute Trust.

Qualifications
  • Registered healthcare professional

  • Basic Life Support (BLS).
  • Level 6/7 clinical skills or equivalent or willingness to complete.

  • Level 7 clinical skills or equivalent competencies.
  • Independent non-medical prescribing.
  • Minor Illness qualification or equivalent.
Knowledge and Skills
  • Professional knowledge acquired through a recognised qualification, enabling registration with a healthcare regulatory body.
