About The Business UnitWestern Plains Correctional Centre is a modern, maximum-security prison focussed on rehabilitation, housing up to 1248 people in custody. About The RoleThe Assessment and Transition Co-ordinator (ATC) forms part of the multi-disciplinary Offender Services team, which is responsible for the development, co-ordination and delivery of a range of programs and services across several specialist portfolios: transition and reintegration, family engagement, disability, diversity and complex care.
The ATC is responsible for the administration of the Corrections Victoria Reintegration Pathway, including co-ordination and facilitation of prisoner risk and needs assessments, transition and discharge planning, parole application processes, and referrals to a range of pre and post release services.
Some Of Your Duties Will Include- Support the delivery of the Corrections Victoria Reintegration Pathway in a prison environment.
- Administer prisoner risk and needs assessment tools to effectively identify rehabilitation and reintegration requirements, making referrals to relevant programs and services as required.
- Work collaboratively with multi-disciplinary teams to develop integrated and effective transition and other specialist support plans, including actively involving and motivating prisoners to engage in goal setting and program/service participation.

About YouTo be successful in this role, you will have:
- Demonstrated understanding of the specific transition and reintegration issues and needs of prisoners.
- Demonstrated ability to administer risk and/or welfare assessment tools.
- Effective stakeholder management skills.
- Ability to communicate sensitively and effectively with diverse cohorts, especially Aboriginal and Torres Strait Islander communities.
- Demonstrated capability to work with people who are difficult to engage individually, and in group settings.
- A tertiary qualification in an appropriate discipline is highly desirable.
- Demonstrated skills and experience in human services, welfare practice or related fields are highly regarded.
