Support Coordinator (COS)
We have a rare opportunity to welcome an experienced Support Coordinator to join our values- driven organisation. This critical role provides the opportunity to engage directly with our customers in a professional and meaningful capacity, allowing them to access the right support and participate more fully in the community. Within our Community Disability team, you will be working to deliver one-on-one services to our customers, coordinating a range of supports in both funded and mainstream areas.
About Us
Founded in 1992, ConnectAbility Australia has proudly provided a broad range of personalised supports and services for people with disabilities, throughout Newcastle, Lake Macquarie, Port Stephens, Maitland, other Lower Hunter local government areas and on the Central Coast.
In 2023, ConnectAbility Australia acquired Early Links inclusion Support. An EarlyInterventionandDisabilityInclusion Support Service offering Allied Health support for people of all ages with a disability in the Lower and Upper Hunter regions.
As a Coordinator of Support in our organisation, under the guidance of your Team Leader, you will be:
- Assisting with advocacy on behalf of the customer and their families/carers.
- Coordinating a range of supports for customers within the office, the community or in the customers home.
- Implementing and participating where needed in the NDIS planning and review of individualised NDIS plans to assist customers to achieve their goals.
- Developing customer / participant resilience in their own network and community.
- Continuing to build internal and external support networks at ConnectAbility.
- Maintaining knowledge of other organisational programs and team members for support referrals.
To succeed in this role, you will hold:
- Solid experience working with adults and/or children with disabilities and their families
- A demonstrated ability to develop strong, professional relationships with customers and their families and hold exceptional communication skills.
- The ability to problem solve, show initiative and contribute to continuous improvement.
- Outstanding interpersonal and networking, negotiation and consultation skills.
- A solid understanding of and experience in working within the NDIS framework.
- The ability to work autonomously and independently, manage a full case load and meet deadlines.
- A thorough understanding of the values and philosophies that underpin a person-centred and strengths-based approach.
- Qualifications and/ or relevant experience in disability, social work, education, leisure and recreation or a related discipline (preferred).
You will also hold a valid Working with Children Check and NDIS worker screening check (or the ability to obtain these) and a current Driver's License.
