What You'll Be Doing

This position is responsible for providing specialist Social Work service to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.

The Senior Social Worker is part of the Primary and Community Health Social Work Department and supports vulnerable families with children aged 0-5 in the Campbelltown and Camden LGA's through psychosocial assessment, support and intervention. The role supports the Sustaining NSW Families Child and Family Health Nursing team at Rosemeadow Community Health Centre. It collaborates with internal and external stakeholders to provide service collaboration, case management and review, using a strengths-based, trauma-informed, culturally safe approach. The Senior Social Worker supports other Social Work clinicians, Social Work students and leads Quality Improvement Projects across the wider department.

SWSLHD Primary & Community Health provides a comprehensive range of community-based services across South Western Sydney, including prevention, early intervention, assessment, treatment, health maintenance and continuing care. The team supports diverse populations with specialised programs for children, young people, older adults and at-risk communities.

With over 800 dedicated staff, the service provides high-quality care close to where people live and offers opportunities for professional growth and meaningful community impact.

Qualifications & Requirements
  • A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).
  • Describe a time when you managed a particularly complex case involving a patient with multiple health conditions, how you applied advanced reasoning and independent judgement, and the outcome.
  • Tell us about a quality improvement initiative you led within a multidisciplinary team, how you incorporated evidence-based practice, and the impact on service delivery or patient wellbeing.
  • Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
