Overview

Allambi Care is seeking a motivated Case Worker to join the Fostering and Permanency team in Charlestown, NSW 2290. This full-time role supports the safety, welfare and wellbeing of children and young people in the foster care program through effective case work and direct support.

Responsibilities
  • Promote the safety, welfare and wellbeing of children and young people in the foster care program through case work and support.
  • Maintain client records and case management systems according to policies, procedures, standards and legislation.
  • Attend supervision meetings and ongoing training as required by management.
  • Advocate for clients and promote their best interests; provide support to a child or young person in care and to their carer families (e.g., home visits, trauma-informed care education, behaviour support strategies, referrals to therapeutic services).
  • Develop and implement case plans, leaving care plans and after-care plans with relevant stakeholders within required time frames.
  • Conduct case reviews to ensure goals are addressed or updated; develop and implement behaviour management and support plans; monitor and collect data on strategies.
  • Coordinate birth family contact in accordance with court orders and care plans, and liaise with carers, birth families and other professionals.
  • Prepare organisational reports and Court materials (e.g., S82 reports, affidavits) to a high standard and timeliness.
  • Liaise with Case Manager, carer families and other service providers to meet the needs of children and young people in care.
  • Contribute to a strengths-based, child-focused practice throughout all work undertaken.
  • Ensure timely response to concerns about safety, welfare and wellbeing of children and young people in care.
  • Promote Allambi Care values and participate in team activities and interagency events as required.
Minimum Requirements
  • Diploma in Community Services, social work, social science, welfare or related discipline (or equivalent experience).
  • Experience in Case Management and/or working with families or children and young people.
  • Understanding of OOHC stakeholders' needs and issues; excellent verbal and written communication skills.
  • Experience with Microsoft Office applications.
  • Knowledge of Needs Based Framework, TCI concepts and strategies.
  • Current Working with Children Check (NSW) and current NDIS Worker Check; current First Aid Certificate (Provide First Aid HLTAID003) and National Police Check.
  • Valid driver's license and comprehensively insured vehicle.
