Overview

Transition Care Program Case Manager (Occupational Therapist, Social Worker, Physiotherapist, Speech Pathologist, Dietitian Level 1/2/ Registered Nurse) – temporary full time position at NSW Health. This is a community role covering the Sydney Local Health District geographical area. The Transition Care Program (TACP) provides low intensity therapies and assistance with activities of daily living to eligible hospital inpatients returning home within up to 12 weeks.

The Transition Care Case Manager ensures that local program therapies, services, documentation, communication, and administrative requirements meet the needs of the client and relevant SLHD, NSW Ministry of Health, and Commonwealth Transition Care guidelines. The role is based at Concord Hospital with possible work from other SLHD locations (RPA and Canterbury Hospitals) and may involve mobile/community work using your own car.

Ideal Candidate
  • Current NSW Drivers Licence (P1 and P2 acceptable).
  • Tertiary qualifications in Nursing or Allied Health (specifically Occupational Therapy, Physiotherapy, Speech Pathology, Social Work or Dietetics) with registration to the relevant professional body (AHPRA or appropriate association).
  • Experience in assessment, care planning, monitoring and review of aged care clients.
Responsibilities
  • Coordinate local program therapies, services, documentation, and communication to meet client needs within Transition Care guidelines.
  • Support transition of inpatients to safe home living with goal-directed interventions for up to 12 weeks.
  • Engage with NSW Health, SLHD, and Commonwealth Transition Care guidelines and ensure compliance.
