Are you passionate about making a meaningful difference in the lives of women experiencing homelessness and mental health challenges? Our client is seeking an experienced and compassionate Case Manager to join a dedicated team and provide high-quality, person-centred support within a biopsychosocial framework.
This role plays a key part in empowering women to achieve stability, recover their mental health, and transition into secure housing. This role is a Fixed Term 12 month contract.
About the Role
As a Case Manager, you will:
- Deliver trauma-informed and strengths-based case management services to women with complex needs, including homelessness, domestic and family violence, and mental health challenges.
- Undertake comprehensive intake assessments, risk assessments, and collaboratively plan support strategies with clients.
- Provide mental health support using focussed psychological strategies such as motivational interviewing and CBT techniques.
- Advocate for clients by liaising with housing providers, health professionals, and community services.
- Lead purposeful transition planning to assist clients in securing sustainable housing and ongoing support networks.
- Maintain accurate and timely case notes, reports, and client records.
- Work within a collaborative, multidisciplinary team while also exercising autonomy and initiative.
- Promote culturally responsive practice, engaging effectively with women from diverse backgrounds including Aboriginal and Torres Strait Islander women, CALD communities, LGBTQIA+ clients, and women with disabilities.
About You
To succeed in this role, you will bring:
- Relevant qualifications in Social Work, Psychology, Social Sciences, Community Services, or a related discipline.
- Demonstrated experience in case management with individuals experiencing homelessness, mental health challenges, and/or domestic and family violence.
- Strong knowledge of risk management frameworks, duty of care obligations, and trauma-informed practice.
- Excellent advocacy and relationship-building skills with clients and external stakeholders.
- Ability to balance independence with collaborative teamwork.
- High level of cultural awareness and commitment to inclusive practice.
- Proficiency in maintaining records, preparing reports, and working with case management systems.

Essential Requirements
- Current Working with Children Check.
- National Police Check.
- Verification of qualifications.
- Current First Aid Certificate (or willingness to obtain).
- Valid driver's licence.
