As the Aboriginal Health Worker (AHW), you will work collaboratively with healthcare service providers across the District to ensure Aboriginal people have appropriate access to healthcare. You will also provide health promotion and education programs appropriate to the needs of the Aboriginal community, with a primary focus on the Chronic Disease for Aboriginal People Model of Care.

The Out of Home Care Health Pathway Program is delivered as a partnership between NSW Health and the Department of Communities and Justice (DCJ) for the health response for children and young people in statutory Out of Home Care in NSW. The Aboriginal Health Worker (AHW) will form part of a collaborative, multidisciplinary team providing care to children and young people 0-17years living in Out of Home Care. The AHW will facilitate, with other members of the team, community development initiatives The AHW will assist in improving the health of Aboriginal families through the provision of high quality, culturally sensitive care, based on primary health care principles, provided in partnership with Aboriginal people/communities. The position will liaise between the clinical stream, major government and non-government stakeholders. All staff are expected to act as an appropriate and effective role model and promote culture and supporting practices that reflect the NSW Health CORE values of Collaboration, Openness, Respect and Empowerment through demonstrated behaviours and interactions with patients, clients and employees For more information and to view the position classification, see the Position Description here.
