We are seeking a compassionate and experienced Case Manager with a strong background in Child Protection and the NDIS to support children and young people with disabilities living in out-of-home care. In this vital role, you will assess individual needs, develop and coordinate comprehensive care plans, and work collaboratively with families, caregivers, and service providers to ensure each young person is supported in a safe, nurturing, and inclusive environment. Your expertise will help drive positive outcomes and promote safety, stability, and inclusion.

Description:

We are seeking a skilled and compassionate Case Manager with demonstrated experience in case management, child protection, youth work, and NDIS coordination. The ideal candidate will be familiar with trauma-informed care principles and behavioural management strategies, and possess the emotional resilience, communication skills, and critical thinking necessary to advocate effectively for children and young people in out-of-home care.

Key Responsibilities and Skills:

  • Case Management & Support: Coordinate comprehensive, individualised care plans for children and young people with disabilities, ensuring their needs are met in a safe, supportive, and inclusive environment.
  • Advocacy: Act as a strong advocate for young people, ensuring their voices are heard and their rights upheld.
  • Collaboration: Work collaboratively with families, stakeholders, and third-party service providers to deliver cohesive, wraparound support.
  • Communication & Problem Solving: Demonstrate strong interpersonal, communication, and critical thinking skills to navigate complex situations and support positive outcomes.
  • Leadership & Team Management: Lead and support a team of senior support workers, providing direction, motivation, and oversight to ensure a cohesive and high-performing team.
  • Quality Assurance: Oversee the implementation of best practices and ensure all care and support services align with organisational policies and industry standards.
  • Staff Development: Promote ongoing professional development through training, mentoring, and a culture of continuous improvement.
  • Compliance: Ensure adherence to all relevant legislation, accreditation requirements, and regulatory frameworks across out-of-home care services.
  • Emotional Resilience & Team Collaboration: Maintain a calm, solution-focused approach under pressure while contributing positively to a collaborative team environment.

Skills and Experiences:

Required Skills And Experience
  • Qualifications: A degree in Social Work/ Community Services and/or relevant qualifications in a human services framework
  • Experience with working with children and young people within child protection, additional experience with working with children with a disability and NDIS systems
  • Experience: Demonstrated and proven experience in case management or similar role, preferably within disability support or out-of-home care settings.
  • Sector Knowledge: A deep understanding of best practices, regulations, and trends within the out of home care industry.
  • Leadership Skills: Strong leadership abilities, with the capacity to inspire and motivate a diverse team to achieve common goals.
  • Communication: Excellent verbal and written communication skills, allowing you to effectively collaborate with internal and external stakeholders.
  • Organizational Skills: Exceptional organizational abilities, including the ability to prioritize tasks, manage resources effectively, and meet deadlines.

You will also need:
  • Positive Notice Working with Children (blue card), or exemption,
  • NDIS Worker Screening Clearance (yellow card), or exemption,
  • LCS2 or the ability to obtain prior to commencing,
  • Clean and current Queensland driver licence
