Overview

Join to apply for the NDIS Support Coordinators role at Illawarra Mercury. This is a full-time, 12-month contract position working 38 hours per week from Monday to Friday. Flexibility to work from home twice a week. Work from North Parramatta, Leichhardt, Campbelltown or Gosford sites as required to the client's place (travel reimbursed). Starting from SCHADS 4.1 $44.58 per hour (depending on experience) plus salary packaging up to $18,550 plus Super.


Responsibilities
  • As The Support Coordinator, Your Role Specifically Will Explore mainstream, community, advocate, informal and NDIS provider options with participants so they are engaged in the decision making of choices and options available.
  • Support participants to establish their MyID account, navigate the NDIS Portal, negotiate services to be provided and support participants to establish service agreements with providers of their choice.
  • Negotiate services and prices as part of any quotable supports as decided by the participant.
  • Arrange any relevant assessments to determine the nature and type of funding required (e.g. complex home modifications).
  • Liaise with Plan Managers on funding, support types, payments etc. where relevant.
  • Link participants to mainstream and/or community services (e.g. housing, education, transport, health) as required.
  • Sustain and enhance participants\' capacity to coordinate supports, self-direct and manage supports and participate in the community, including providing assistance to resolve problems, understand responsibilities under service agreements, and change or end a service agreement.

Essential Qualifications
  • Diploma or higher tertiary qualifications in disability/community services/social sciences/allied health and equivalent knowledge/experience.
  • Knowledge of the NDIS Code of Conduct, Practice Standards, Act 2013 and rules.
  • Understanding of the complex needs of people with disability and their families/nominees/authorized representatives.
  • Experience with effective and contemporary disability support models and person-centred approaches.
  • Proven ability to establish active partnerships and achieve outcomes with communities and organisations.
  • Proficiency with Microsoft Office and client database systems.
  • Excellent report writing, time management and record-keeping skills.

Desirable
  • Fluency in a language other than English.
  • Experience in working with First Nations communities and people from Culturally and Linguistically Diverse backgrounds.

Prerequisites to Employment
  • Current driver's licence and willingness to use own or private vehicle for work travel with relevant insurances.
  • NSW Working with Children Check, NDIS Worker Screening Check, National Criminal Record Check.
