Role Description

We're inviting dedicated and experienced Specialist Support Coordinators to join our expanding team in a contractor capacity. As a Specialist Support Coordinator, you will deliver a higher level of support coordination to our most complex participants by:

  • Developing and implementing strategies to help participants achieve their plan goals.
  • Promoting autonomy through education, capacity-building, and informed decision-making.
  • Regularly assessing service effectiveness and making necessary adjustments to enhance outcomes.
  • Connecting participants with a wide range of services and support networks.
  • Advocating a strengths-based approach that fosters independence and personal growth.
  • Managing crisis situations effectively to achieve positive outcomes.
  • Collaborating with multidisciplinary teams to ensure comprehensive support.
  • Keeping accurate records of participant progress and interactions.
  • Preparing detailed reports to track progress, identify areas for improvement, and highlight successes.

Qualifications and Skills

  • Tertiary qualification in Social Work, Psychology, Allied Health, or a related field, with membership (or eligibility for membership) in a relevant professional body.
  • Experience working with individuals with complex needs in the NDIS, government, or community sector, with a strong understanding of service systems and barriers affecting participants.
  • A commitment to professional integrity, equity, and inclusivity.
  • Strong verbal and written communication skills.
  • Demonstrated ability to build positive working relationships with local NDIS providers, mental health services, homelessness support, drug and alcohol services, Child Protection, emergency services, and the forensic system.
  • Problem-solving skills, resilience, and the ability to work effectively in a dynamic environment.
  • Strong organisational and time management skills with a process-oriented mindset.
  • A proactive work ethic and commitment to high-quality service delivery.
  • Proficiency in CRM systems and digital tools for documentation and communication.
