Job type: Casual. Classification: SCHADS Award Level 2 Pay Point 1 + 12% super.
You will- Provide mental health and disability related supports to participants of the National Disability Insurance Scheme. This includes but is not limited to:
- Facilitating capacity building and person centered supports to develop independent living skills within home and community-based settings.
- Undertake community access supports to participate in leisure activities to build social connectedness and increase autonomy to complete community-based activities (e.g. provide support to attend appointments, assistance with shopping, budgeting, meal preparation or daily life activities).
- Implement a strengths-based, recovery-oriented and trauma informed care approach.
- Provide in home care supports (e.g., personal care, hoisting, showering).
- Provide complex and tailored support to participants displaying behaviors of concern.
- Assist with personal care, in home care and supports, hoisting, peg feeding, bowel care, medication assistance and prompting.
- Communicate with key stakeholders including clinical supports to work towards client goals and continuity of care.
- Undertake accurate case notes and maintain client records.
- Maintain client privacy and confidentiality at all times.
- Engage in monthly supervision and regular group reflective practice.
- Be willing to undertake relevant professional development or training opportunities.
- Undertake general house cleaning tasks for or with participants that promote autonomy including but not limited to mopping, vacuuming, dusting, wiping down areas.
- Have the ability to positively engage clients with complex and acute mental health presentations.
- Identify, manage and de-escalate any foreseeable related risk.
- Uphold and maintain professionalism and respectful conduct while representing All In One Support Services.
- Demonstrate strong interpersonal skills, build rapport, motivate, and build positive and professional relationships.
- Utilise IT platforms.
- Maintain a safe working environment, reporting any workplace related risks to the Operations Manager immediately.
- Experience and knowledge working alongside complex mental health and disability presentations.
- Experience working within an NDIS framework and an understanding of core functions of the NDIS.
- Experience in a human services role with the ability to demonstrate excellent interpersonal skills, communication, organisational and engagement skills.
- Experience working in Mental Health or Disability fields.
- Attend scheduled community outreach supports and engage with clients by implementing a recovery-focused approach that aligns to their NDIS plan related goals.
- Be available to undertake a minimum of 30 hours of work per week on an ongoing basis.
- Uphold a professional standard of practice when representing All In One Support Services in the community with clients or when liaising with other services.
- Remain contactable via mobile phone during designated support times and work hours.
- Complete accurate case notes on the same day support is delivered.
- Follow All In One Support Services workplace policies and procedures.
- Tertiary qualification in a human services or related field or Certificate IV or Diploma in a Mental Health, Disability, or Community Services related field. (Preferred).
