Role Description

The Service Coordinator will be responsible for tasks related to participant services, including Supported Independent Living (SIL), Community-based activities, and Short-term accommodations. This position is an integral part of our management team, as the Service Coordinator will play a pivotal role in ensuring smooth service delivery, maintaining compliance with NDIS guidelines, and managing stakeholder relationships.

Key Responsibilities

  • Service Delivery: You will be responsible for overseeing the delivery of services to our participants. This includes assisting participants with the specific support they require to meet their individual needs.
  • Problem Solving:Addressing barriers to engagement, ensuring smooth communication with the participant and their respective care team.
  • Compliance & Standards:Ensuring that all services in Melbourne are delivered in accordance with NDIS guidelines and maintaining compliance with quality and safety standards.
  • Team Leadership:Working closely with a multidisciplinary team, promoting a culture of teamwork and excellence.
  • Reporting:Providing regular reports on service delivery metrics, participant feedback, and any operational challenges to senior management in Melbourne.
  • Rostering:Maintaining and updating the participant's care plan and rostering system.
  • Budget Management:Collaborate with the accounts team to ensure services are delivered within budget in Melbourne and identify opportunities for financial efficiency.

Qualifications

  • Service Coordination and Operations Management skills
  • Interpersonal Skills and Communication abilities
  • Customer Service expertise
  • Strong organizational and time management skills
  • Ability to work effectively in a team
  • Experience working in the disability services sector is a plus
  • Relevant certification or diploma in Community Services or related field
