Afterhours Carer Services Navigator – Wellways Australia

Responsibilities
  • Triages carers contacting Carer Gateway and, where appropriate, implements end-to-end services including intake, registration, assessment and planning.
  • Provides emergency respite care for carers who are experiencing crisis and carer burnout to access urgent support.
  • Supports carers via phone or online channels during afterhours shifts (Monday – Friday 8pm–8am, weekends and public holidays 24 hours).
What you will need
  • Diploma qualifications in a relevant field, plus experience in community services, allied health or carer/family peer roles.
  • Experience in supporting individuals experiencing crisis and/or complex supports.
  • Right to work in Australia; current valid Driver's Licence and ability to travel.
  • NDIS Worker Orientation Module (free online).
  • Satisfactory National Police Records Check and NDIS Worker Screening Check.
  • Working with Children Check (Employee/Paid).
