Job Description

About the role

Seeking a Level 3 Social Worker, who will provide psychosocial assessments and complex discharge planning for two inpatient oncology wards with a large elderly patient population. Work closely with medical teams to deliver patient-centred care in a supportive, part-time role.

Key responsibilities

  • Deliver psychosocial assessments for oncology inpatients
  • Lead complex discharge planning with multidisciplinary teams
  • Provide support to patients, families, and carers

Skills & experience

  • Degree in Social Work
  • Current or eligible AASW registration
  • Strong skills in discharge planning and assessments
  • Fully vaccinated for COVID-19 (including booster)
