Position objectives: The overall role of the HYAP Family Support Worker is to work and support families and young people who may otherwise enter OOHC due to homelessness. They assist in reducing risk in the home by linking families into community supports as directed and providing them with a framework to maintain the connection developed while connected to our program.
This is a support worker level position, not a case worker level position; however it will assist in professional development with some aspects of outreach and fundamental case work.
Main Duties and Responsibilities- Provide support to families and clients in a non-judgmental and therapeutic manner.
- Follow Allambi risk assessment procedures when visiting and meeting families for the first time, and ongoing.
- Transport and support clients and/or family to appointments and other important commitments as outlined in the case plan or directed by management.
- Assist in the completion of safety and risk assessments for each family within the required timeline, including the North Carolina Family Assessment tool where needed.
- Respond to crises in a therapeutic and professional manner, consulting with Management.
- Ensure the Case Worker is immediately informed of any critical incidents, especially child protection issues, and debriefing for workers as needed.
- Ensure detailed incident reports are completed and submitted to the Case Worker within 24 hours.
- Work flexible hours including evenings where needed to support the family and young person.
- Ensure contact is made with the Case Worker prior to each family visit and be available at short notice for shifts as demand dictates.
- Attend and contribute to client planning processes, including case plans, case conferences and other intervention programs.
- Review and assess progress of case plan goals with the Case Worker and discuss barriers to completion.
- Complete the family support worker tool for each family, aligning with goals to work towards.
- Act as an advocate and support for clients and assist them in expressing grievances.
- Attend overview meetings with the case work team to ensure goals are met and families have adequate supports for success in the program.
- Maintain communication of all visits/contacts/meetings with families to the Case Worker via family contact reports; submit within 24 hours where applicable.
- Engage with clients and families to build positive connections.
- Attend ongoing internal and external training as identified by Management Team.
- Promote Allambi Needs Based Restorative Framework (NBRF) culture and work to NBRF ideals.
- Perform the Family Support Worker role across a variety of settings (home, schools, extended family, hospitals, courts, community centres, emergency accommodation).
- Ensure that all family contact/work aligns with case plan goals and complete helpline reports when child protection issues are identified (discuss all helpline notifications with management).
- Collaborate with other NGOs and government departments and maintain positive professional relationships with external services.
- Assist families and young people to gain practical life skills to independently access supports in the community.
- Maintain professional conduct at all times and ensure responsible driving practices in company vehicles.
- Notify the Case Worker and People Support and Culture of any grievances, issues, or concerns regarding employees and families.
- Adhere to Allambi Care policies and procedures; prioritise, organise and complete tasks within allocated time frames; work independently and as part of a team.
- Undertake additional tasks as assigned by the Case Worker, Case Manager and Department Director.
- 3 years working with children and young people in a similar environment to Allambi
- Diploma of Community Services (or working towards a similar qualification)
- Experience in Microsoft Office applications
- Knowledge of NBRF concepts and strategies
- Experience working with people with a disability or challenging behaviours
- Experience working in an outreach setting
- Full-time employment for a fixed term (up to 12 months)
- Salary packaging (salary sacrifice)
- 4 weeks annual leave
- 12 roster days off a year (one per month)
- Monday – Friday work week
