OverviewAlzheimer's WA (AWA) is the leading provider of dementia care services in Western Australia, offering advocacy, education, partnerships, training, consultancy and holistic person-centred care programs. Services include CHSP, HCP and NDIS packages, and day/overnight respite in four homes (Perth, Woodvale, Mandurah, Albany). The Care Coordinator supports people living with dementia in social settings, coordinating individualised support from onboarding through to review. This role is based at Mary Chester House in Shenton Park.
Responsibilities- Monitor contractual hours of services within the allocated budget
- Input and maintain accurate data/records for staff, clients and carers
- Evaluate service delivery to identify gaps and drive continuous improvement
- Assist in formulating and implementing team procedures
- Coordinate referrals to ensure appropriate services, adhering to guidelines, policies and procedures
- Apply a reablement and wellbeing approach to define client goals and capture feedback
- Coordinate support workers and ensure effective staff utilisation
- Provide dementia-specific information to clients and carers
- Coordinate services including social support, in-home respite, groups and day centre services
- Provide feedback to the House Manager on service issues
- With the House Manager, supervise, monitor, support, train and evaluate staff and volunteers
- Assist with recruitment and onboarding as directed by the House Manager
- Process timesheets for staff/volunteers
- Monitor safe working conditions for all community and respite staff
- Collaborate with other AWA departments to ensure best practice
- Liaise with external service providers to facilitate information sharing and promote AWA services
- Tertiary qualification in aged care or community services or extensive, relevant experience
- Previous experience in the health and aged care industry
- Ability to plan and organise workload to meet deadlines
- Empathy and experience working with people living with dementia
- Excellent interpersonal and communication skills, including stakeholder management
- Working knowledge of Microsoft Office applications
- Valid driver's licence
- NDIS Check (or willingness to obtain)
- Up-to-date COVID-19 vaccinations (3 doses)
- Knowledge of services available for people with dementia and their carers
- Experience providing services to people in the community
- Ability to lead a small team
