Graceville Centre at Nambour is an integral part of the healthcare community on the Sunshine Coast. We provide tailored support for people to reimagine their lives by setting and achieving their goals and learning the skills to increase their independence.

Your Role – Supporting & Encouraging Growth: At Lutheran Services, we are looking for compassionate and motivated Support Workers to provide flexible, person-centered support to individuals living with disabilities.

As a Support Worker, you will:

• Provide flexible support tailored to client needs and preferences.
• Assist with personal care and daily living tasks, ensuring dignity and independence.
• Encourage community participation and social engagement for individuals.
• Support skill development and independence-building activities.
• Work collaboratively with case managers and other stakeholders.
• Maintain accurate documentation of client care and interactions.

What You Bring to the Team:

• Certificate III or higher in Individual Support, Disability Support, Aged Care, or Home Care.
• A strong passion for empowering individuals with disabilities.
• Excellent interpersonal skills to build positive relationships with clients and their families.
• Flexibility to work within a structured fortnightly schedule.
• A valid Queensland Driver's Licence.
• A valid NDIS Worker Screening Clearance & Blue Card.

Federal Government legislation now requires all workers within Aged Care or Community Services to provide and maintain an NDIS Worker Screening clearance. This system has replaced the Yellow Card system. This is a mandatory requirement, for anyone working within these industries and a "no card, no start" policy now applies.
